Service Description

Physiotherapy initial assessments in the comfort of your own home. These appointments will take up to 1.5 hours and include: - A comprehensive clinical assessment carried out in your own home, including an in‑depth medical history and collaborative goal setting. - Travel to and from your location (within a 5‑mile radius of Cannington). - Any required follow‑up communication, such as phone calls or written correspondence with your GP, district nurses, or the wider MDT, to support your rehabilitation plan. - A personalised written plan provided after the assessment to guide your next steps.
